Book a repair yourself,
just like your customer.

Pick a form type and your industry. Then click through every step your customer sees, all the way to the finished request. Nothing gets sent.

Your industry

Don't see your industry? The form always shows your own services and prices from RepairCMS. So it also works for payment terminals, sewing machines, cameras, air conditioners, household appliances and much more.

Mail-in

Customers who live far away can mail you their device.

If your customer lives far away, they would usually take their device somewhere else. With mail-in, they send it to you instead. If your booking location is in Germany, they get a prepaid DHL return label by email and pay nothing for shipping. The postage is billed through your own DHL contract.

If RepairCMS creates the label right after booking, your customer also gets a QR code and doesn't need to print anything. Outside Germany or without the add-on, your customer gets a simple PDF label with your address and pays the postage.

One system

Your office, your team and your customer see the same job.

A plain booking form stops at the request. In RepairCMS, one step turns it into a job. Your team works on it from their phone, while your customer follows the repair live in the browser, and soon in their own customer app.

Every request lands here, whether it comes from your website or from the tablet on site. You review it and create the job in one step. Change a price here, and it changes in the form on your website too.

Your team prints the label

Your team checks in the device on their phone and prints the label straight to a Brother label printer. Later, your technician scans the label and has the job on their phone, without walking over to the computer.

Your repair request form carries your logo and asks what you need to know.

When your customer describes the fault and sends photos right away, you save yourself a callback. You decide which steps your form has and in what order, from brand and model to add-on services. You set each field to required or optional.

Form types

Every way to reach you gets its own form.

Website, tablet on site, intake by your team, warranty claim. You create as many forms as you need, each based on one of the four form types. All requests land in your service requests.

For workshops and service centers

Repair Leads

Your customer types the repair into the search field on your website, sees the price up front and books. If the diagnosis finds more, they get your quote first and decide for themselves. You can also share the full form by link, QR code or on social media.

Kiosk

Your customer checks in on a tablet on site while your team keeps repairing. Then the form resets itself for the next customer.

Service Agent

At intake, your team records the job step by step, offers every customer your add-on services and prints the job receipt and device label right away.

For manufacturers and service networks

RMA and warranty

Instead of an email with loose attachments, you get a warranty claim with proof of purchase and photos. Your team reviews it in the service requests, just like any other request.

B2B Partner Portal

Your partners book their repairs themselves in their own portal, instead of sending you error-prone lists. An add-on from the Team plan.

Custom workflows

If your service network has its own rules for RMA and warranty, we build a workflow tailored to you as an Enterprise project.

We build your repair request form into your website, or into a new one if you like.

Frequently asked questions about the repair request form

What should a repair request form ask for?

A repair request form asks for what you need at intake: the device's brand and model, the fault, your customer's contact details and address, terms and privacy, and a signature if you want one. In the Form Builder you set the steps and their order yourself and make each field required or optional. If your customer sends photos of the fault right away, you save yourself a callback. How to organize intake in your business afterwards is covered in our article on the repair intake process. Every step and its settings are covered in our help article All steps of your form at a glance.

Can I embed the repair request form on my website?

Yes. You embed your form directly on your website and can also share it by link, QR code or on social media. In the form, your customer searches for the repair they need, sees the price up front and books. If the diagnosis finds more, they get your quote first and decide for themselves. Every booking lands as a service request in RepairCMS, and one step turns it into a job. For the steps to add the code, see Publish, share and add your form to your website, and for WordPress, Elementor, Wix, Jimdo and websites built with AI, see Add your repair form to WordPress, Wix and other websites.

Is there a template for an RMA form?

Yes, RMA and warranty is one of the four form types in the Form Builder and comes with the right steps built in: device, fault, serial number, purchase date, invoice number and photos. Instead of an email with loose attachments, you get a warranty claim with proof of purchase and photos. Your team reviews it in the service requests like any other request. You adjust fields and steps yourself and create as many forms as you need. You can see each step of the RMA form in our help under RMA and warranty claim forms.

Does the repair request form work for bikes, watches or coffee machines?

Yes, because your customer searches the form for the service they need instead of clicking through long device lists. You maintain services and prices centrally in RepairCMS, and when you change a price there, it changes in the form on your website too. So the form works for smartphones and tablets, computers, bikes and e-bikes, watches and coffee machines just as well as for manufacturers that run their own service. You can see which industries we support on our page about repair business software.

Can my customer mail in their device for repair?

Yes. When your customer chooses mail-in in the form, they get a prepaid DHL return label by email if you use the Repair Logistics add-on. Repair Logistics costs €29 a month excl. VAT at the launch price, and you can add it from the Business plan. This includes creating 100 labels a month, and each additional label costs €0.07 excl. VAT. You pay the postage through your own DHL contract, and the DHL label requires a booking location in Germany. Outside Germany or without the add-on, your customer gets a simple PDF label with your address and pays the postage. You can read how the connection works on our page about Repair Logistics. To let customers mail in their device, see Edit the steps of your form.

How much does the Form Builder cost?

The Ultimate Repair Form Builder is included from the Team plan, so you pay nothing extra. Team costs €99 a month excl. VAT when billed annually, or €110 a month excl. VAT when billed monthly. You can compare all plans on our pricing page.
